Terms and Conditions
- Our plan is not insurance or qualified under the Affordable Care Act.
- Once signed up, you are committed for the entire year. You may cancel at any time but will not receive a refund.
- Payment for Dental treatment(s) is/are not included in the plan and must be paid at the time of service.
- Routine X-rays do not include Pano/FMX/CT imaging (3-5 years).
- Membership does not include products or whitening services, and does not cover procedures completed by specialists outside our practice.
- Membership plan cannot be combined with insurance, discounts or other offers.
